The laryngeal mask, or laryngeal mask vent, is a concave sleeve sealed at low pressure around the entrance of the larynx to allow spontaneous breathing or positive pressure ventilation.
Laryngeal mask has become a common airway for routine anesthesia in pediatric and adult general anesthesia. The implantation method varies with different products.
Insertion method
At present, a variety of laryngeal masks have been applied in clinical practice, among which Classic laryngeal mask, Ambu laryngeal mask, SLIPA laryngeal mask and I-gel capsuleless laryngeal mask are widely used.During the placement of the laryngeal mask, the patient should be adjusted to the head and neck extension position with good exposure to the neck, and the gas in the mask capsule should be evacuated in advance, and the laryngeal mask should be placed by the method recommended by the manufacturer.Taking Classic laryngeal mask as an example, its insertion method is as follows:
Before inserting the tube, check whether the connection of each part of the laryngeal mask is reliable and whether the sleeve is leaking.Use proper lubrication on the back of the spoon - shaped sleeve of the larynx cover.Because the laryngeal mask does not enter the trachea, it is less irritating to the patient and can be placed under moderate sedation and pharyngeal surface anesthesia.Can not cooperate with the application of muscle relaxant after placement.
It is usually placed by blind probing.In supine position, the operator pushes the mandible or lower lip with the left hand to open the mouth of the patient, and holds the laryngeal mask with the right hand towards the direction of the patient's jaw, and places the top of the laryngeal mask into the mouth towards the direction of the patient's hard palate;Push down the posterior pharyngeal wall along the median line of the tongue until it can no longer push forward when it meets resistance.Do not insert the mask vertically into the mouth to prevent the front end of the mask from disintegrating or curling and making it difficult to maintain the correct orientation.After implantation, inflate the capsule and check the position of the laryngeal mask.
